Instructions for EKPC Pole Attachment Request Application

Please complete the information below and provide a plan view sketch or drawing that accurately depicts the proposed configuration of the proposed attachment and any other details that may be important. Hand sketches or computerized drawings/maps are both acceptable. Please email this form and all accompanying maps, sketches, drawings, etc. to TRANSMISSION_ENGINEERING@ekpc.coop with THIRD PARTY ATTACHMENT REQUEST in the subject line. Confirmation that your request has been received should be sent to you by return email within 2 business days. Please also direct any additional questions to TRANSMISSION_ENGINEERING@ekpc.coop.

  • Corporate Safety Program Documentation
  • Past 3 years of audited financial statements
  • Planned construction commencement date and project schedule
  • Statement ensuring the design attachments meets the minimum NESC Grade B construction strength requirements.
Installer Information:

The following information must also be submitted before your application will be processed:

  • Applicable Certificate of Insurance for the construction of the proposed attachment
  • Corporate Safety Program Documentation

* if no company has yet been selected to construct the attached asset, construction specifications, insurance limits, and safety requirements that will be used by the Owner for contractor solicitation/award may be submitted instead.
